How to organise care for a parent after hospital discharge
Discharge packs get lost. WhatsApp threads miss doses. Use one shared care space so the first week home is calmer for everyone.
The first 72 hours
- Photograph every discharge sheet and store it in the document vault
- Enter or scan every medication before the first evening dose
- Create or update the SOS card with allergies and GP details
- Invite at least one other family member while you still have energy

Scan labels into one shared medication record. Note dose changes vs what they were on before admission.
Book follow-ups before the week gets busy
Add GP, physio, and specialist appointments to the shared calendar so siblings do not double-book or miss them.
Assign who owns each evening for seven days
Invite siblings and carers to the care circle. The activity feed replaces “did you give the evening dose?” texts.
Before the follow-up appointment
